For questions in the form, "Which of the following is least supported?" or "not supported", might the AAMC make one of the answer choices something that was not mentioned at all in the passage? If that is the case, is it the correct answer (i.e., is it "not/least supported" because it was not mentioned at all?)? Or should we cross out an answer choice if it is not ever mentioned?

That can depend very much on the wording of the question. If the wording is "which one of these PASSAGE ASSERTIONS" is not supported by the author, and the answer choice is not a passage assertion, then you should eliminate it since that is not responding to what is being asked for in the question.
